Forms on-line


 

Responding to the needs of those who come to court without an attorney and need help completing court forms, Register of Probate Richard Iannella officially opened a self-help center in November, 2000. It is the first center of its kind in Massachusetts.

 

In May of 2005, the center was renamed The Robert L. Kavin Resource Center in memory of Bob Kavin, a Registry employee credited as the brainchild behind many of the Registry's effort to assist unrepresented litigants. You can read more about Kavin by clicking here

 

An average of 20 people each day - more than 5,000 a year - make use of the information, materials and other resources available at the center, located in the Registry offices at 24 New Chardon Street, Boston. The center is staffed daily from 9AM-4:30PM, and assistance is provided on a first-come, first-served basis.

Eligible Registry clients may also visit the volunteer "Lawyer for the Day" for formal legal advice and assistance with required court paperwork.

 

Registry forms currently available on-line:

The most commonly used court forms are listed below. In many types of court cases, several different forms will need to be completed in order for the case to proceed. If the form that you need is not listed here, it is likely a rarely-used form, and you must contact the Registry to obtain it.

 

Some of these on-line forms have been prepared using the Rich Text Format (RTF). RTF files can be opened in most word processing programs and operating systems, and feature shaded fields that can be filled-in by using your computer keyboard before printing. 

 

All other forms are in the widely-used Adobe Acrobat (PDF) format, and can be  printed out and completed by hand. A free Adobe Acrobat reader is available by clicking on the link below.
